Our approach

Support should have a purpose

HealthHeit focuses on participation, skill development, stability and the right level of help for the person's needs today. Staff never take over because a task is faster.

Six stages, one standard

  1. 1
    Teach

Show the skill once, in the real setting, with the real item: the posted schedule, the pay stub, the bus app.

  2. 2
    Prompt

Use the least help that works: a picture or gesture first, then one verbal prompt. Wait before adding more.

  3. 3
    Practice

The person does the step, repeatedly, on real days.

  4. 4
    Observe

See what the person does without help. Done before staff arrived counts as independent.

  5. 5
    Fade

When a skill holds, step back: fewer prompts, then a phone check, then monitoring only.

  6. 6
    Monitor

Check after changes: a new job, a new schedule, a move. Re-teach only what slipped.

Rights first, and written down

  • Goals come from the signed IPP, in its wording. Nothing is invented.
  • The person controls their own money, relationships, medical decisions and community access. We coach; we do not sign, hold or decide.
  • Declining a task is a right. We record it neutrally, never as a failure.
  • Every entry is written so a reader could see what a camera would have seen. Statements are attributed to who said them.

Routines, coping and stability

Some people need support with routines, frustration, transitions, communication or conflict. Within the authorized plan we use predictable routines, positive reinforcement, prompting, redirection and coping strategies, and we coordinate with qualified behavioral or clinical professionals when they are involved. Clinical treatment stays with clinicians.
