Measurement
How we know it's working
Every skill we support is recorded with the level of help the person actually needed. Progress means less help over time, sustained. That is what we show service coordinators, not adjectives.
Independent
Done without staff help, including before staff arrived.
Gesture
A picture, a point, a visual cue. Then wait.
Verbal
One reminder or question at a time.
Physical
Hands-on help, only when appropriate, and documented.
Declined
The person's choice not to do a task. A right, recorded neutrally.
Each person has a written trajectory
| Domain | Baseline | 90-day target | When support fades | When support grows |
|---|---|---|---|---|
| Morning medication routine | From the record at month end | Started from the picture on 4 of 5 staffed days | Held 8 weeks: shorten the morning visit; keep verification | Two weeks below baseline: restore full presence |
| Budget reconciliation | From the record at month end | Weekly reconciliation with a verbal prompt or less | Held 60 days: move to biweekly review | Unplanned shortfall: return to weekly review |
Support changes follow these written rules, so a person is never cut off because they are "doing well," and never kept dependent because no one checked.
Outcomes from our records
Real, dated, de-identified. No names, no counts, no invented statistics.
Employment
An individual moved from job search to competitive employment. Support is now built around keeping the job and protecting benefits.
Money and benefits
Individuals opened their own bank accounts, set up direct deposit, secured CalFresh and had SSI payments resumed, while keeping control of their own money.
Daily routines
Daily routines that had rarely been completed became consistent once support was daily and structured, and related self-care routines followed.
Community and transit
Dial-A-Ride and bus routes learned and used. A crochet class, church, a wellness center peer group and volunteering, chosen by the person, with staff presence tracked and fading.
Progress happened alongside the support provided. We report it as association with the support, not as proof that support alone caused it. That honesty is what makes the record credible.
